Nursing Care Plan for patients whose Safety is Threated
A nursing care plan identifies the existing and potential needs of a patient. The plan provides platforms for effective communications of the nurses, patients, and other healthcare stakeholders to enhance outcome. It is important in establishing consistency and quality of healthcare. The plan has four stages. The first stage is the nursing diagnosis which involves assessment of patient’s health status, medical results, and preparing diagnostic reports. The second stage is the outcome stage. At this stage the nurse will outline the expected short-term and long-term patient outcomes. Thirdly, the intervention stage will involve the treatment plans that the nurse designs for the patient. At this stage, the nurse will design preventive measures for cases like falling. The last stage is the evaluation stage that documents the outcome of nursing interventions. The nurse evaluates the intervention plans to assess effectiveness and efficacy through patient outcome.
